Before you apply

Have your California seller’s permit number ready. Dr. Choice Pharmaco does not issue seller’s permits. If you do not have one, apply with the California Department of Tax and Fee Administration (CDTFA) first.
Complete and sign a resale certificate, such as California form CDTFA-230 General Resale Certificate, and upload it with your application or email it to us afterward.
If a Dr. Choice Pharmaco sales representative is helping you, enter their Sales Rep ID.

What happens next

Our team reviews every application. Wholesale pricing is enabled only after approval and receipt of a valid resale certificate.
Once approved, the authorized buyer receives an access email and signs in with the regular Log in button on this site.
Orders are reviewed by our team before they are confirmed.
